A penis extender applies a gentle, sustained stretch along the shaft. Held under steady tension over a long period, the tissue gradually adapts and lengthens — the same principle used elsewhere in medicine. How to wear it.
Phallosan Forte works differently from a rigid rod extender. The glans is held in a soft, vacuum-sealed protector cap, which is then connected to a sleeve and an adjustable belt worn around the waist. The belt applies a gentle, even pull to the side — so the device sits flat and is discreet under clothing.
- Create the vacuum seal so the protector cap holds the glans comfortably and securely.
- Attach the sleeve and fasten the belt around your waist, routing the device to the left or right.
- Set the tension using the colour-coded strength bands — start gentle and build up over time.
- Wear it for a few hours a day (the study averaged about 6 hours); comfortable enough for long sessions and even overnight once accustomed.

A German urological clinical study
Following the launch of Phallosan Forte in 1999, a German urological healthcare facility ran a clinical study to assess whether the device could meaningfully lengthen the penis. Of 26 men initially enrolled (aged 20–68), 24 were included in the analysis. Participants wore the device for an average of 6 hours a day (range 2–10 hours) over 6 months.
- Flaccid length increased markedly by 3 months and significantly more by 6 months — with greater gains in the second three months than the first.
- Erect length followed the same pattern: higher at 3 months, higher again at 6 months.
- Girth also increased at 3 and 6 months, with gains described as evenly distributed.
- Curvature: one participant with a curved penis showed noticeably reduced curvature after 6 months.
- Satisfaction: 22 of 24 reported higher overall satisfaction; improvements were also reported by some in erection quality, duration, hardness and sensitivity.
In 2006, The Journal of Sexual Medicine reported the clinic's findings, concluding that with continuous use men can increase penis length by wearing the extender for six months or more. Individual results vary; figures are averages, not a single result.

Do extenders really work?
Penis extension devices have some scientific validity. In theory, sustained traction should stimulate new tissue growth in the penis, and — unlike penis lengthening surgery (ligamentolysis), which only increases flaccid length — a traction device can increase both flaccid and erect length.

A pilot phase-II prospective study of a penile extender in men with a "short penis." Wearing the device 4–6 hours daily over 6 months produced a measurable mean increase in flaccid length (~1.3–1.7 cm), and the device was well tolerated.

A contemporary review of 15 studies (1,000+ patients) found modern traction devices achieved penile length gains averaging 1.5–2.3 cm with high adherence, alongside curvature reduction in Peyronie's disease. A noted limitation across the field: long-term persistence of gains after stopping is not yet well established.
Read the review ↗The honest caveat: results require a high level of commitment (often 1,000+ hours of wear), and many extenders are uncomfortable. In practice a significant permanent gain is hard to achieve, and many users stop after a month or two. The key to results is finding a device comfortable enough to wear for the maximum time under tension — which is exactly the problem Phallosan's vacuum-and-belt design sets out to solve, replacing the loop-around-the-head pressure point with a sealed vacuum that transmits tension via a waist belt.

Pros & cons
Extender vs filler.
A lot of patience is required to achieve results with Phallosan Forte, or any penis extender. While the science behind extenders is sound, in practice many men give up due to the amount of commitment required.
The Androfill filler procedure, by contrast, provides an instant increase in girth — which can be dramatic depending on how much filler is used. The trade-off is cost: Androfill is considerably more expensive than buying a penis extender.
